・Information affected: Email address, membership number, membership type, name, and desired player's name entered by the customer

Cause of Occurrence

When creating the change request form, it was incorrectly set to a state where other responses could be viewed. Additionally, a double-check of this setting was not performed.

Damage Confirmed to Date

We have not received any reports of damage from customers.

Future Response

We have sent an email to the affected customers to report on this incident and offer our apologies. We will thoroughly implement double-checks when creating and configuring various forms to prevent recurrence.
