Please note that this change request is only for those who applied online. Those who applied using paper forms such as at the stadium, by FAX, or by mail are not eligible for changes. Please be aware of this in advance.

How to Operate
[1] Enter "Membership Number" and select "Date of Birth" to log in
[2] Select [Add Parking / Change Uniform Procedure]
[3] Select the content to change
1. If you want to change the uniform size, check the box
2. If you want to change the uniform name & number, select from the pull-down menu
3. After selecting your desired changes, select [Next]
[4] Confirm application details and select [Confirm]
[5] Application complete
Notes
・If multiple change requests are confirmed, the latest application content will be applied.
・If new players are announced during the request period, the selection options will be updated as needed.
・Under no circumstances will change requests be accepted after the change request period has passed. Please be aware of this in advance.
・Season Ticket (U18) holders and regular members who purchased a uniform as a paid option are not eligible for change requests. Please be aware of this in advance.
