Elementary School Individual Participation: (1) Date of participation, (2) Grade, Name (If a school member, also include course), (3) Parent/Guardian's mobile contact number
Elementary School "3on3": (1) Team name, (2) Grade, Participants' names (all team members), (3) Parent/Guardian's mobile contact number
※The application deadline is two days before the event.
【Injury Response】
First aid will be provided, but subsequent treatment will be at the participant's expense.
Thank you for your understanding and cooperation.
【Cancellation】
In case of cancellation, a decision will be made and communicated two hours before the start of the event on the day.
