Adult Individual Participation: (1) Date of participation, (2) Name, (3) Mobile contact number
Elementary School Individual Participation: (1) Date of participation, (2) Grade, Name (If a school student, also provide the course), (3) Guardian's mobile contact number
Elementary School "3-on-3": (1) Team name, (2) Grade, Participant names (all team members), (3) Guardian's mobile contact number
*Application deadline is 2 days before the event.
[Injury Response]
First aid will be provided, but subsequent treatment will be at the customer's expense.

[Cancellation]
In case of cancellation, we will make a decision and notify you at least 2 hours before the start time on the day of the event.
