Elementary School Individual Participation: (1) Date of participation, (2) Grade, Name (If a school student, also specify the course), (3) Parent's mobile phone number
Elementary School "3-on-3": (1) Team name, (2) Grade, Participant names (all team members), (3) Parent's mobile phone number
*Application deadline is two days before the event.
[Injury Response]
First aid will be provided, but subsequent medical expenses will be the responsibility of the participant.

[Cancellation]
In case of cancellation, a decision will be made and communicated at least two hours before the start of the event.
