Choshi School Elementary School Individual Futsal (3/20) Event Announcement

We will be holding individual futsal sessions for 1st to 6th graders at Choshi Sports Town Gymnasium. Individual futsal is where participants form teams and enjoy playing matches together. Please take this opportunity to join us!

Saturday, March 20 (National Holiday)
1) 13:00 - 14:30 Lower Grades (1st - 3rd Grade Elementary School Students) Registration: 12:30 - 12:50
2) 15:00 - 16:30 Upper Grades (4th - 6th Grade Elementary School Students) Registration: 14:30 - 14:50

Choshi Sports Town Gymnasium
(1600 Nojiri-cho, Choshi-shi, Chiba)

■Participation Fee
500 yen (tax included)
*Please try to bring exact change.
*Participation prizes and MVP prizes will be awarded.

School students, General public

■Number of Participants
1) Lower Grades (approx. 30 students in total from 1st-3rd grade elementary school)
2) Upper Grades (approx. 30 students in total from 4th-6th grade elementary school)
*Applications for this event will be accepted on a first-come, first-served basis. If the capacity is reached and you cannot participate, we will contact the guardians. Please be aware of this in advance.

■Application Deadline
Until 23:59 on Wednesday, March 17, 2021

■What to Bring
Ball, water bottle, athletic wear, indoor shoes (must be for indoor use)
*Please be sure to write your name on all your belongings.
*Guardians who come to observe should also bring slippers or indoor shoes.

■Notes
・Please arrive at the venue on time on the day of the event.
・Please refrain from parking in nearby commercial facilities or on the street.
・First aid will be provided for any injuries sustained during practice.
・After that, it will be your responsibility, and the club will not be held liable. For school students, sports insurance will cover incidents.

■Coronavirus Countermeasures Guidelines
Please be sure to take your temperature on the day of participation.
On the day of participation, adults (18 years and older) with a fever of 37.0℃ or higher, or children (under 18 years old) with a fever of 37.5°C or higher, will not be allowed to participate. Also, if you experience difficulty breathing, fatigue, malaise, or abnormalities in taste or smell, you will not be allowed to participate.
If you or someone living with you has experienced any of the above symptoms within one week prior to the day of participation, you will not be allowed to participate.
Those who have been requested to self-isolate by their school or workplace will not be allowed to participate.
Please strictly adhere to the following when participating:

1) Be able to disclose your activity record for the past two weeks (when, where, and what you were doing) to the club.
2) Participants understand the risks and confirm infection control measures.
3) Always wear a mask when entering the venue.
4) Maintain social distancing and avoid crowding. Follow instructions from staff if given.
5) If you feel unwell during participation, report it immediately and go home.
6) Prepare your own bottle/water bottle at home and do not use others' (be sure to write your name clearly to prevent mix-ups).
7) Practice cough etiquette even on the pitch; do not spit or blow your nose with your hands.
8) Promptly leave and go home after the event ends.
